A code is one line on an overview. Its meaning comes from the conversation: what was done, why is it there and which possible next steps were discussed?
Reading the extra markers
A fixed NZa amount is not always the complete invoice amount. Appendix 2 explains when other costs or a higher maximum may apply.
Only for performances marked with one asterisk. These are necessary, separately purchased dental-technical work and materials specifically attributable to the performance — not ordinary consumables. The costs must be itemised transparently.
Only for performances marked with two asterisks. These are specifically attributable costs of external bacteriological laboratory testing and must also be shown separately.
A maximum tariff may be increased by no more than 10%, and only under a written agreement between the provider and insurer. It must apply to the insured person and include the relevant performance.
Estimate and invoice
The Dutch Oral Care Regulation defines how an estimate and the final invoice must be itemised.
From €250, the practice must provide a non-binding estimate before treatment. Below €250, you may request one. It should show performances, quantities, tariffs, the total and any extra costs.
From €500, an oral estimate is not sufficient. Between €250 and €499.99, an oral estimate is possible only by agreement; the total and your consent must be recorded.
For external dental-technical or laboratory work, you may request the supplier invoice. A practice website must also publish its current material and technical price list.
